Annual Patent and Financial Reports must be submitted by an authorized Certifying Official "on line", meaning electronically, and in hard copy via mail. Investigators are required to submit annual Progress Reports electronically and in hard copy.
Certifying Officials should submit signed Patent Forms and Financial Reports to:
