We are grateful to the friends of The Leukemia & Lymphoma Society (LLS) who support our lifesaving work by making provisions in their wills, trusts, insurance policies or retirement accounts. The following is suggested language for including LLS in an estate plan:
For a specific amount:
I give to The Leukemia & Lymphoma Society Inc., 1311 Mamaroneck Ave., White Plains, NY 10605 (specific dollar amount or percentage of my estate or specified asset).
For a residuary amount after other bequests are made:
I give to The Leukemia & Lymphoma Society Inc., 1311 Mamaroneck Ave., White Plains, NY 10605 all (or a percentage) of the rest, residue, and remainder of my estate.
For a contingent bequest if you outlive other beneficiaries:
If (other named beneficiary) does not survive me, I give to The Leukemia & Lymphoma Society Inc., 1311 Mamaroneck Ave., White Plains, NY 10605 all (or a percentage) of my estate.
You can make your gift without restriction or direct it to research, to patient services or to a particular chapter. Because LLS has a single 501(c)(3) designation, all documents should refer to the Home Office address, 1311 Mamaroneck Ave., Suite 310, White Plains, NY. Our federal tax identification number is 13-5644916.
