Welcome to The Leukemia & Lymphoma Society Co-Pay Assistance Program, Provider Portal.

The Co-pay Assistance program provides patients financial assistance for health insurance payments and with blood cancer treatment co-pay obligations. Co-Pay applications can be submitted by calling (877) 557-2672 from your physician facility or submitted online through this secure web-based process.

We have created this site to be available to you as a method to access financial assistance for your patients in an easy manner. This application process should take approximately 5 minutes to complete.

If this is your first time visiting the Co-Pay Assistance Provider Portal and you would like to register, please click Physician Registration. If you would like to begin the application process for your patient, please click Start New Patient Application.

Please note that your provider tax id number is required to complete the registration process. Once approved, you will receive a username and password (via email typically within 5 minutes) for the site allowing future utilization of this web based service.
