Material Order Form

Thank you for the work you do to benefit The Leukemia & Lymphoma Society® through the Light The Night® Walk.  The following is a list of various materials you can order to assist you and/or your Team in fundraising efforts.  Please review the list and enter quantities of each item that you need. When you click 'Submit' at the bottom of the page, your request will be sent to the Western PA/WV Chapter Light the Night department.

   Fundraising Materials
Please insert the quantity of item you would like in the box before the item name.
Small Sponsor Envelope - Envelope used to collect donations (each team member should have one.)
Brochure Holder - Used to display brochures.
Posters - Posters with event details to promote the walk.
Fundraising Thermometer - Poster to track fundraising total.
Team Recruitment DVD - Can be used to promote & motivate team members
Brochures - Great way to recruit individuals or other team members.
Dress Down Day Stickers - Great office fundraiser! Co-workers can make a donation to dress casually for a minimum $2 fee.
Paper Balloons (Packs of 50 and UPC coded) - Balloons can be sold at local establishments or displayed in work place featuring participating team members. Usually sold for $1.
Coin Canisters - Can be distributed at local establishments to collect money.
Large Collection Envelopes for Team Captains - Used to collect money from team members (each team should turn in a large envelope).
Plushland Bear Catalog - Great way to raise money by selling bears.
Luminary Order Sheets - 4 forms per sheet - Luminaries line the walk route and are in honor or memory of a loved one. The minimum donation for each luminary counts toward your fundraising goal. All luminary money should be sent in a separate envelope one week prior to the walk.
Donor Form/ Receipt - 3 receipts per sheet - For anyone who makes a donation & requests a receipt.
Newsletter/Church Bulletin Insert - Small flyers that can be added to mailings and publications to get the work out about the walk and/or you walk Team
